Universal Media Release Form

I acknowledge and consent that any video, images, and/or audio recordings recorded as a participant in this Joni and Friends activity that contain my image and/or voice may be used without restriction by Joni and Friends for any purpose, including promotional and marketing use, on social media, and for public broadcast.

Joni and Friends shall have unconditional ownership of and broadcast rights to such video, images, and/or sound recordings. These conditions apply to any submitted media, such as video and audio recordings, and digital or printed photographs provided by me as a an on-camera participant for Joni and Friends use.

I hereby release and discharge Joni and Friends and its licensees from any claims of damage or
cause of action resulting from usage of these videos, images, and/or audio recordings. I agree that the consent provided herein is irrevocable.

IF THE PARTICIPANT IS UNDER THE AGE OF 18, A PARENT OR LEGAL GUARDIAN MUST ALSO ACCEPT THIS THIS CONSENT / AGREEMENT AND STIPULATIONS ABOVE.
