• Be prepared to complete everyone’s application in one sitting (one page per individual). You will not be able to save your information.
• The first application page is for Head of Household (parent o legal guardian) who is over 18 and will serve as primary contact. It’s intended that the Head of Household complete applications for all attendees.
• Payment is not required with your application. You will receive payment/fundraising information once your application is approved.

Application Checklist

You will be asked about the following for each person applying with you.

• Personal information: name, date of birth, etc.
• Allergies
• Medical information: disability/medical condition(s), medications, accommodations needed
• Insurance policy carrier and policy number
• Physician information: name, phone number, address
• Personal Care Attendant name (this role can be part of a family application OR apply separately)
• Two emergency contacts for family unit: name, email address, phone number
• T-Shirt size
