• Be prepared to complete everyone’s application in one sitting (one page per individual). You will not be able to save your information.
• The first application page is for Head of Household (parent or legal guardian) who is over 18 and will serve as primary contact. It’s intended that the Head of Household complete applications for all attendees.
• Payment is not required with your application. You will receive payment/fundraising information once your application is approved.
• Background Check: Joni and Friends is committed to the safety and well-being of every participant. Each applicant (18 and older) must complete an online background check and will be asked questions regarding previous accusations of child abuse and drug usage.

Application Checklist

You will be asked about the following for each person applying with you.

• Personal information: name, date of birth, etc.
• Allergies
• Medical information: disability/medical condition(s), medications, accommodations needed
• Insurance policy carrier and policy number
• Physician information: name, phone number, address
• Personal Care Attendant name (this role can be part of a family application OR apply separately)
• Two emergency contacts for family unit: name, email address, phone number
• T-Shirt size
