The Respite Initiative

By filling out this application, I agree to attend this event. I understand my commitment to serve at this event allows a family to be moved off the waitlist and experience a night of respite. Should I need to cancel for any unforeseen reasons, I understand the impact this will have on the church and the families served.

As a requirement of this registration application, I acknowledge and consent that any photographs, video and audio recordings that contain my image and/or voice, and the image and/or voice of any children for whom I am parent or guardian, may be used without restriction by Joni and Friends for any ministry purpose, including public broadcast. This consent applies to each adult person included in this registration as well. We hereby release and discharge Joni and Friends and its licensees from any claims of damage or cause of action resulting from usage of these photographs, video and/or audio recordings.
